We are looking for a dedicated, experienced and energetic Registered Nurse to join us as a Healthcare Coordinator at our Cle du Cap and Noordhoek Manor facilities within the Southern Suburbs.  Duties include but is not limited to the following:

  • Provide primary healthcare services to residents attending the village clinic
  • Receive information from the Administrator and complete care plans (if necessary) medication scripts and clinic requirements 
  • Ensure all clinic visits, incidents and emergencies are accurately recorded on our electronic charting system
  • Keep detailed records of resident clinic visits and services provided
  • Conduct annual wellness visits to all village residents
  • Monitor nursing/care notes and shift handovers on our electronic charting system weekly
  • Review care plans with the Shift Leader when necessary and assist in the Healthcare as required
  • Screen sub-acute patients in hospital prior to admission, assess suitability and explain the admissions process
  • Liaise with the Medical Liaison, Shift Leader and doctor to finalize admissions
  • Refer problems or potential concerns to the Nursing Services Manager and healthcare team
  • Support the Shift Leader with healthcare tasks as required
  • Maintain clinic equipment to ensure all remains in good working order
  • Check and control the stock and equipment of the emergency trolley as per the check list
  • Manage and coordinate home-based care, including scheduling carers and handling billing processes
  • Assist in evaluating residents for possible transfers to assisted living or healthcare
  • Manage emergency care response procedure in conjunction with the Nursing Services Manager
  • Provide medication management and oversee safe practices
  • Assist with administrative tasks as required
  • Train and orientate new staff members


Requirements:

Qualifications - Registered Nurse qualification, SANC registered

Experience and Knowledge – Experience in medical/surgical nursing (sub-acute and long-term care), dementia care, frail care and palliative care.

Specific Requirements

  • Own Transport
  • No criminal record

Working Hours - Normal working days are Mondays to Fridays from 07h00 until 17h00. You will be required to be on standby one weekend per month to assist in the healthcare if needed

The salary on offer is market related and will be dependent on qualifications, experience and other market-related factors.
